The structure is:
starting in a hospital i am your doctor - leads to a flashback which leads to me - leads to us in your basement being raped with etc - leads to end of the flashback and waking up in the hospital - you are my doctor.
You wake up in a hospital confused, you where told you where in a car crash and came out of a coma, however, your nurse looks familiar. then you have a flashback and well she wasn't only just your nurse but something else for the night hehe
